Repeated infusions of interleukin-2 in patients with metastatic malignant melanoma. The effects of repeated infusions of interleukin-2 (IL-2) on malignant melanoma were investigated in 35 evaluable patients, 18 of whom were given IL-2 alone; 10 were given IL-2 in combination with lymphokine-activated killer (LAK) cells; and seven received LAK cells alone. Twenty-six of these patients had melanoma with cutaneous lymph node (nodal) metastases (stage M1), and the rest (stage M0) had distant metastases. There was no evidence that IL-2 administration stimulated host-reactive cells in vitro. Patients were given IL-2 administered by bolus injection six times a day for 4 consecutive days followed by a 2-week rest period. Responses were measured clinically by radiologic and by biologic (IL-2 assay) response criteria. Correlative studies by microcytotoxicity assay and fluorescent-activated cell sorter analysis showed that the tumor burden in nodal and subcutaneous lesions was decreased in 13 of 14 patients who had both a biochemical and a radiologic response. Responses correlated with the concentration of IL-2 in biologic assays performed on circulating lymphocytes. Adverse effects included the characteristic flu-like symptoms of fever, chills, and myalgia/arthralgia; marked transient capillary leakage (serum protein and creatine phosphokinase elevations greater than 10 times normal); an intense pleuritic chest pain, resulting in chest tube drainage; and, in 3 patients, pulmonary embolus and pulmonary hypertension, manifested clinically by dyspnea and dramatic increase in pulmonary vascular resistance. The overall response rate was 45% (4 of 9). Of the 18 patients who received IL-2 alone, 2 of 10 and 8 of 8 had a positive biochemical and radiologic response, respectively. Of the 7 patients given IL-2 and LAK cells, all had a positive biochemical response (p = 0.0007), and 6 of 7 had a positive radiological response, though with less frequency than for IL-2 alone (p = 0.13). Thus IL-2 is active against metastatic melanoma. Toxicities noted with greater frequency than in the IL-2 monotherapy arm were inflammatory responses.
