The aim of this study was to compare the accuracy of cephalometric and protractor measurements with that of a goniometric technique in the analysis of facial profile. The study included 40 male and 40 female subjects aged 18 to 25 years. A three-dimensional (3-D) laser scanner was used to record a reference cast. A linear paired-protrusion was made between the maxilla and the mandible. The upper and lower incisors were aligned on a straight line perpendicular to the Frankfort horizontal plane (FH). A second cast was made with a defined protractor and a second straight line was made between the upper and the lower incisors. A third cast was made with a goniometer and the profile angle was measured. Subsequently the recorded casts were superimposed on the reference cast. The discrepancy between the actual and the ideal profile position in the plane was expressed as the percentage of the actual distance to the nearest ideal point on the profile. The measurements made by the linear paired-protrusion (27.5 +/- 4.1%) and by the protractor (26.9 +/- 4.3%) showed low discrepancy when compared with the measurements by the goniometer (27.8 +/- 4.8%). The measurements made by the protractor and by the laser scanner had a larger discrepancy (32.0 +/- 4.0% and 32.8 +/- 4.7%, respectively). The results of this study show that the main discrepancy of the protractor measurement of a defined line in the profile occurs in the anterior segment. The protractor and the linear paired-protrusion were more accurate than the goniometer.
